- Acetaminophen (Tylenol) — even though it's common, it can be hard on the liver, especially at higher doses or with regular use
- NSAIDs (like ibuprofen or naproxen) — these can also be risky with liver involvement
- Tramadol is an opioid-type medication, so mixing it with other pain relievers needs careful oversight
The most important step is to check with both your rheumatologist and your hepatologist (liver specialist) before adding anything new — even something over-the-counter. What's safe for most people may not be safe with PBC.
