For itching-related discomfort (which can feel painful), there are several treatment options doctors may consider:

- Antihistamines – such as Benadryl, Zyrtec, Allegra, or Claritin, available over the counter
- Cholestyramine – a bile acid resin considered a first-line treatment for itch
- Rifampin – a second-line option that works on itch signals in the brain
- Opioid antagonists – like naltrexone, which may reduce itch but need close monitoring
- SSRIs – sertraline has shown some benefit for PBC-related itch in small studies

Always speak with your doctor before starting or changing any pain or itch management routine, especially since some medications can affect liver function or interact with ursodiol.

Gabapentin worked for my neuropathy but I didn’t take it long term. There’s negative side effects with gaba.

Thanks for replying. My hepatologist said only ibuprofen, no Tylenol, but I see a lot of doctors contradict that. That’s why I end up taking nothing. Gabapentin didn’t work for me. I will try lidocaine. Thanks.

My doctor has said that Tylenol is okay for me but only 4 capsules (up to 2000 mg) in 24 hour period.
I’m using lidocaine patches and cream. I have severe osteoarthritis(bone on bone) migraines and still waiting on rheumatologist referral.
It seems that the Gabapentin (up to 900 mg) that I am taking for my anxiety/ neuropathy is ok.
I’m still at stage 1 though so I would definitely talk to your physician as many pain killers are very hard on the liver.
