A positive AMA test is a strong indicator of PBC, but it doesn't automatically confirm the diagnosis on its own. Around 95 percent of people with PBC have positive AMA results, and having AMA means there's a high probability you have PBC.
However, to be diagnosed with PBC, you typically need to meet at least two of these three criteria:
* Elevated ALP levels (at least 1.5 times the upper limit of normal)
* Presence of AMAs with a titer of 1:40 or higher
* Liver biopsy showing liver damage associated with PBC
Your AMA level of 90 is significant, but your doctor will also look at your liver enzyme levels, particularly ALP, and consider whether you have any symptoms.
Regarding the positive ANA: Around half of people with PBC also have positive ANA results. ANAs are more common in people with PBC who don't have AMAs, but they can appear alongside AMAs too. ANAs are found in many autoimmune conditions, so they're less specific to PBC than AMAs.
If you're AMA-positive but have normal liver enzymes and no symptoms, your doctor may recommend regular monitoring every six to 12 months rather than an immediate PBC diagnosis. This helps catch any liver changes early.
