Members shared their alkaline phosphatase (ALP) numbers in response to a question about how high levels can get and which medications help, with reported levels ranging from 68 to over 870. Several members described how Urso (ursodiol) brought their numbers down significantly, with some adding Iqirvo or Livdelzi for further improvement, though side effects like hair loss, nausea, and heart palpitations were noted by some. A recurring theme was the importance of monitoring bloodwork closely, keeping a food diary to track diet's impact on liver numbers, and staying proactive with medical teams when side effects arise.

My ALP is extremely high. My recent bloodwork (2 days ago) was at 412, which is the highest it's ever been! The lowest was 270 in May 2025. I'm taking Urosidiol 300mg 3x daily and Seladelpar (aka Livdelzi) 10mg. Livdelzi has helped keep my levels stable, but what has really made the difference is diet. I fell off my liver-friendly diet, and I can see the drastic change reflected in my recent high reading. I'm now doing a juicing fast (with greens, veggies, and fruits) to stabilize and lower the ALP. The AST is slightly elevated, and ALT is normal. The issue for me is the cirrhosis and inflammation. Keeping a food diary really helps to pinpoint the highs and the lows of test readings. Hope this is helpful to someone! Do your research, know your body, stay positive, and give yourself grace....πŸ™πŸ½
