That "I've already run a marathon and it's 7am" feeling is very real with PBC — and you're definitely not alone in experiencing it!

Waking up with achy, heavy legs is closely tied to the fatigue that comes with PBC. One myPBCteam member described it perfectly: feeling heavy, like being in a force field, even after resting. Part of what's happening in your muscles is linked to how PBC affects energy production. The body ends up relying more on anaerobic energy (think: no oxygen involved), which produces higher levels of lactic acid in the muscles. That buildup is what causes that heavy, achy, worn-out feeling — even without any physical activity.

A few things that may be making it worse include:

- Poor sleep — over half of people with PBC experience sleep disorders
- Itching at night disrupting rest
- Nutrient deficiencies — PBC can affect absorption of vitamins A, D, E, and K
- Anemia — if present, it can significantly worsen muscle fatigue It's worth bringing this up with your doctor, as they can check for deficiencies or other contributing factors. Gentle movement, like short walks or stretching, can also surprisingly help improve energy and reduce that heavy feeling over time — even when it's the last thing you feel like doing!
