@A myPBCteam Member. My professional advice is to speak to your hepatologist about taking it. There’s so many factors that influence the answer. It’s not a black and white answer. Only your liver specialist will be able to answer your question. The pharmacist can’t even answer because he/she doesn’t know the details of your history and the pharmacist is a generalist.

Managing arthritis pain alongside PBC is something worth discussing carefully with your doctor, especially since the liver plays a key role in processing medications — including paracetamol (acetaminophen).
Paracetamol is generally considered safer for the liver than anti-inflammatory painkillers like NSAIDs (e.g., ibuprofen), which can be harder on the liver. However, with PBC, even paracetamol needs to be used cautiously and only at the lowest effective dose. Here are some important points to keep in mind:
- Always check with your doctor or pharmacist first before taking any painkiller regularly
- Stick to the lowest effective dose — don't exceed the recommended amount
- Avoid alcohol entirely when taking paracetamol, as this significantly increases liver stress
- NSAIDs like ibuprofen are generally best avoided with PBC due to the risk of liver damage
Alongside medication, some non-drug approaches may also help with pain, such as:
- Gentle regular exercise like walking
- Using a heating pad for localised discomfort (15–20 minutes at a time)
- Eating an anti-inflammatory diet rich in fruits and vegetables
Your liver specialist or hepatologist is the best person to guide you on the safest pain relief option for your specific situation.
