That's a really important question. Stage 4 PBC means there is advanced liver scarring (cirrhosis), and treatment options do become more limited at this stage — but there are still options worth discussing with your doctor.

Here's what's generally known about newer treatments at this stage:

- Ursodiol (UDCA) is still used and can help, though it's most effective in earlier stages
- Elafibranor (Iqirvo) and seladelpar (Livdelzi) are newer PPAR agonist medications — however, neither is suitable for people with *decompensated* cirrhosis (when the liver can no longer function adequately)
- Liver transplant becomes the most effective treatment option when the liver is failing at this stage, and most people with PBC who receive one have a good outlook The key distinction your doctor will look at is whether your cirrhosis is *compensated* (liver still functioning) or *decompensated* (liver struggling to function). This makes a big difference in which treatments are safe for you.

It's really worth having a detailed conversation with a hepatologist (liver specialist) about where you are and what options fit your specific situation.
