I was told Tylenol was ok– and to avoid Advil and Aleve as much as possible-(once in a while was ok.) I was told CBD was ok, I was told that the liver has no pain receptors, and pain from that area is the result of pressure on the surrounding area. Heat pad helps also.

What types of pain can occur with PBC, and how can it be managed?

PBC can cause several types of pain due to liver inflammation and complications. The most common is discomfort in the upper right abdomen where your liver sits. Many members describe it as dull, aching, or stabbing pain. Sometimes it spreads to your back, shoulder blades, or neck.

The pain happens because of:
- Liver inflammation and swelling
- Ascites (fluid buildup in your abdomen)
- Splenomegaly (enlarged spleen)
- Pressure on surrounding tissues

One myPBCteam member shared they get pain when their liver touches the counter while cooking. Others describe a hardening sensation and dull pain, especially when eating. For pain management, your doctor may recommend several approaches:

Medications:
- Over-the-counter pain relievers like ibuprofen or aspirin for mild pain
- Prescribed medications to treat PBC complications (cholesterol-lowering drugs, bone loss treatments, portal hypertension medications)
- Your regular PBC medications, which help by improving bile movement and reducing liver inflammation

At-home strategies:
- Use a heating pad for 15-20 minutes at a time to relax muscles and ease discomfort
- Eat a healthy diet rich in fruits, vegetables, and whole grains
- Try smaller, more frequent meals to reduce abdominal discomfort
- Get regular exercise, even just walking 30 minutes daily
- Eliminate alcohol
